CPT Code for Bone Grafts - Spinal Fusion

Coding a spinal fusion case where the surgeon used allograft material and part of the bone taken from a laminectomy to do a posterior fusion. Are both bone grafts (allograft and autograft) billed and are they billed per level fused?
